Salary

$94,469 to $111,765 pro-rata, per annum

Please note that access to salaries beyond $105,158 is subject to completion of a relevant Graduate Diploma or application to the personal upgrade scheme in accordance with Schedule 8 of the Allied Health Professionals Public Sector Unions Wages Agreement 2019

Period of Employment

Permanent, part-time, day work, working 60.8 hours per fortnight commencing I July 2021.

Huon & Community Health Centre

Duties

  • Within a primary health care framework, provide a social work service to individuals, families, carers, groups and communities with a focus on developing evidence informed services that are reflective of community needs.

  • Promote community awareness in relation to building community resilience and work with other agencies, organisations and wider community to develop an integrated approach to delivering health services.

  • Be responsible as a sole practitioner for the Community Health Social Work Service in a Community Health Centre and within the Huon Valley community.

  • Provide a casework, case coordination and case management service to individuals and families with complex needs, drawing on a broad range of therapeutic models.

  • Assess the social and health requirements of diverse communities and implement strategies and programs to meet the health needs in line with the direction of the Agency.

Essential Requirements

  • Degree in Social Work giving eligibility of membership to the Australian Association of Social Workers.

  • Current Driver’s Licence

The Head of the State Service has determined that the person nominated for this job is to satisfy a pre‑employment check before taking up the appointment, on promotion or transfer. The following checks are to be conducted:

1. Conviction checks in the following areas:

a) Crimes of Violence
b) Sex Related Offences
c) Serious Drug Offences
d) Crimes involving dishonesty
e) Serious traffic offences
